I am a preferred provider with:

• Medicare
• Blue Cross
• Tricare

I can bill most commercial insurance and offer a sliding scale based on income.

I can see you for an initial visit without a physician referral, but if you wish to continue treatment, a physician must approve your Plan of Treatment.
